PLEASE READ: Group Class Waiver
I have requested the right to participate in dog class/es at A Good Dog’s Life training facility. Such an enrollment having been made of my own initiative, I understand that attendance at dog class/es is not without risk to myself or my dog. Some of the dogs, although appearing healthy and being handled with the greatest amount of care and foresight, may act out unexpectedly in an unfamiliar environment.

I fully understand that if I am to participate in a dog class, I accept such inherent risks to myself and my canine(s).

Now, therefore, in consideration for permitting me to participate in such training and/or activities, I hereby waive, release, and forever discharge Carolina Canine, Incorporated (aka:A Good Dog’s Life) and its owners, their agents and employees from any and all claims, while on the grounds or surrounding areas.

I also agree to indemnify Carolina Canine, Incorporated (aka:A Good Dog’s Life), its owners, their employees and agents, from all loss and expense, including but not limited to damages, legal expenses and cost of defense, in any manner, arising from the action of any dog including my own as well as my use of the property.

I further agree to pay any veterinary and/or medical expenses incurred as a result of injury caused by my dog/s. I give A Good Dog's Life permission to seek veterinary care at a veterinarian of their choice if they deem it necessary for my dog/s at by expense. However, I will not hold A Good Dog's Life, its owners, their employees and agents, responsible if they fail to seek veterinary care. I further understand that payment made for class registration is not refundable for any reason.

I understand that I am solely responsible for controlling my dog/s. No liability with be accepted by A Good Dog's Life or the instructors thereof, for any damage or injury suffered by a dog owner or hi.her dog/s, or third party person/s or dog/s, or damage to third party property, arising through any cause, whilst attending training classes or other functions promoted by A Good Dog's Life.

Facility rules that participant further agrees to:
· All canines must be in good health.
· All canines shall only be off lead only while instructors work calls for such action or the instructor has permitted participants to do so.
· All canines must be kept under control at all times.
· No females in heat MUST wear a diaper and be kept in a crate while not working. The final decision is up to your instructor, so please contact her.
· You MUST clean up after your dog, brushing your dog prior to class is appreciated.
· OUR GOAL IS TO OFFER A SAFE ENVIRONMENT FOR TRAINING: If, in the opinion of the instructor, any dog brought to any training class or functions promoted by A Good Dog's Life, is unfit for training, is ill or is suffering from any disease, then that dog bay either be refused admission or be removed from the training session.

I HAVE READ AND AGREE TO THE ABOVE WAIVER*

For the safety of all participants, we must have a current copy of your dog's vaccination record or proof of titer on file. Please send a copy of your dog's vaccination record along with your $40 deposit or bring them with you to the first class or workshop.
